The following is a text version of the original publication. -- To: Division of Disability and Rehabilitative Services Providers From: Nicole Norvell, Director, Division of Disability and Rehabilitative Services Re: Wellness Coordination Provider Enrollment and Recipient Health Scores Date: January 28, 2014 This is a reminder that the Division of Disability and Rehabilitative Services (DDRS) is currently accepting applications from providers who would like to add Wellness Coordination as a service they render. As previously announced, The Centers for Medicare & Medicaid Services (CMS) is currently reviewing the proposed new service, and we anticipate that the service will be available in the near future. Providers will not be added to the consumer pick list until CMS has approved Wellness Coordination as a service on the Community Integration and Habilitation (CIH) Waiver. An announcement will be distributed upon approval. However, to expedite the provider enrollment process, DDRS is encouraging providers to submit their applications now. Information on how to apply is outlined in the Wellness Coordination Provider Update, which was released on December 17, 2013.
Also, to assure access to the new Wellness Coordination service for eligible participants, DDRS will forward each individual’s raw health score to his/her current residential habilitation and case management provider. DDRS is requesting that the residential habilitation providers and the waiver case managers review the current raw health score. If there is a concern that the current score is not accurate the waiver case manager should submit a Budget Review Questionnaire (BRQ) via INsite, thoroughly outlining all areas of concern or disagreement. Please attach supporting collateral reports (such as doctor’s statements, incident reports, diagnostic updates, etc.). A personal allocation specialist will review your request and make needed updates to ICAP addendum medical frequency and intensity scores, and reply with details of the findings via the BRQ.
