The Connxxions Program started on November 1, 2004 and we recently celebrated fifteen years of serving the IDD/MH population. Fifteen years ago, most mental health providers believed you could not have an intellectual disability and a mental health concern. Therefore, programs for this dual population were almost non-existent. Today, Meridian’s Connxxions Program serves 1,600+ patients in 37 counties and has more than 60 developmental disability provider partners. We are on the cutting-edge of intensive community based interventions and dual diagnosis care in general. We are also, fully engaged in improving areas of outcome measures, integrated care, and trauma work for the population we serve. As a state-wide leader in integrated care we truly value the opportunity to join you as we learn more about how to serve those with intellectual or developmental challenges. Meridian Health Services is proud to be a co-presenter of the INARF Annual Conference this year and continue our commitment to supporting wellness in this often misunderstood and overlooked population.
We hope you will take advantage of our dual diagnosis learning track as you attend the this year’s conference. As the first outpatient program to achieve accreditation from NADD, an association for persons with developmental disabilities and mental health needs, we understand the unique needs this intersection creates for individuals. While at Conference, please take the opportunity to stop by our booth outside Ditslear on Wednesday and booth #24 in the Expo Hall on Thursday. Our staff will be available both days passing out goodies and answering questions about how our dual diagnosis program works in partnership with other established services.
