The Family and Social Services Administration, Division of Disability and Rehabilitative Services, Deaf and Hard of Hearing Services (DHHS) have released a Request For Funding (RFF) for community agencies throughout the state to provide case management services to carry out the Increasing Independence Program. Case managers provide community support services to enhance independence to individuals who are Deaf or Hard of Hearing who have significant hearing loss. The primary objective of this project is to provide statewide access to high quality, comprehensive case management services to individuals who are Deaf or Hard of Hearing in the most cost effective manner possible. DHHS encourages respondents, in their responses to the RFF, to be as creative as possible regarding strategies for meeting this objective and carrying out the scope of work described in the RFF. DHHS also encourage responses from entities that have capacity to provide statewide access to case management services for the target population, as well as responses from entities that have capacity to serve a specific geographic area only. Cost efficiency for the State will be a consideration in determining whether a grant(s) will be awarded based on responses to the RFF.
The RFF and due dates are available on the DHHS Web page by clicking here. Any questions regarding this RFF must be submitted in electronic format to Rhonda.Marcum@fssa.in.gov no later than 4:30 p.m. Eastern Standard Time on September 8, 2017. Questions received after 4:30 p.m. may not be considered. Please keep questions brief and of high priority. Comments are closed.
