The following is a text version of the original publication. -- To: Bureau of Developmental Disabilities Approved Providers From: Nicole Norvell, Director, Division of Disability and Rehabilitative Services RE: Living Arrangement for Individuals on the Community Integration and Habilitation Waiver Date: April 1, 2015 The Bureau of Developmental Disabilities (BDDS) is committed to ensuring that Individualized Support Teams (IST) continue to have ongoing communication that assists participants in the development and implementation of the individual's Individualized Support Plan (ISP). In order to ensure that information is accurately reflected in the States’ case management system Advocare, BDDS has worked to make available through the Advocare Provider Portal, the opportunity for Providers who are listed on a current Notice of Action (NOA) to view the individual’s current Living Arrangement. Documents on the Division of Disability and Rehabilitative Services’ Announcements page outline the steps that a provider may take in order to view this information and contact the participant’s case manager should the information be inaccurate. If you are a current BDDS provider and have not yet signed up to participate in the Advocare Provider Portal you may do so by following the instructions also on the DDRS’ Announcements page. BDDS appreciates your continued commitment to those individuals receiving services through the Community Integration and Habilitation Waiver.
