The following is a text version of the original publication. -- To: Division of Disability and Rehabilitative Services waiver consumers From: Division of Disability and Rehabilitative Services Re: Community Integration & Habilitation Waiver Amendment – Wellness Coordination Update Date: December 17, 2013 As previously announced, Indiana's Division of Disability and Rehabilitative Services (DDRS), is adding Wellness Coordination as a new service to the Community Integration and Habilitation Waiver (CIH). The Centers for Medicare & Medicaid Services (CMS) is currently reviewing the proposed new service, and we anticipate that the service will be available during the first quarter of the 2014 calendar year. Wellness Coordination is a service designed for consumers who require the ongoing help of a nurse (RN/LPN) to properly coordinate their medical services. The three tiers (levels) of service are based on health scores that come from the State’s objective based allocation process. Wellness Coordination is available for consumers whose health-related needs extend beyond the services provided through routine doctor/health care visits. As medical events occur and/or as a consumer’s medical needs change, the Individualized Support Team is expected to seek reassessment for potential revision to the health score and to ensure utilization of the appropriate tier of service.
DDRS is currently finalizing the process for provider enrollment for the new service, as well as the process for adding Wellness Coordination into the service plans for those eligible consumers with health scores of five or higher. This additional service will not change or limit the amount of any other CIH waiver services, including those the consumer already receives. Comments are closed.
