The Bureau of Developmental Disabilities Services and the Bureau of Quality Improvement Services are asking providers to submit information regarding the re-opening of services following the COVID-19 pandemic via a standard form. While we greatly appreciate the information that we have received from providers thus far, completing and submitting this form will ensure that we have consistent and comprehensive information from every provider on their re-opening process. To complete this form you will be asked to provide:
