The following is a text version of the original publication. -- To: Division of Disability and Rehabilitative Services Providers From: Nicole Norvell, Director, Division of Disability and Rehabilitative Services Re: Wellness Coordination Update Date: May 30, 2014 In March 2014, the Division of Disability and Rehabilitative Services (DDRS) posted a document entitled “Wellness Coordination: A Guide for Providers” on the DDRS Announcements webpage. In the document there was a typo that affected service delivery requirements for individuals who are deemed eligible for Tier III service. The correct definition of the Tier III service per the CMS approved Service Definition is as follows: Tier III: Health care needs require at least twice weekly consultation/review with RN/LPN including face to face visits once a week
The Wellness Coordination Provider Guide has been removed from the DDRS website and has been replaced with a Frequently Asked Questions document and the Service Definition for Wellness Coordination. Both are available on the DDRS Announcements webpage. The CMS approved CIH 5th Amendment that includes Wellness Coordination as a service under the CIH waiver is also posted to the DDRS website. The State believes that these documents will answer many of the questions and will provide clarity to those providing Wellness Coordination services.
