-- To: Waiver participants, families and stakeholders From: Nicole Norvell, Director, Division of Disability and Rehabilitative Services Re: CIH Transform Town Hall Meetings Date: September 4, 2015
The Division of Disability and Rehabilitative Services (DDRS) is holding stakeholder town hall meetings throughout the month of September in order to gain feedback about CIH Transform, “A multi-year effort to engage stakeholders and advance services for individuals with disabilities.” This presentation will be given at each stakeholder meeting to outline the discussions that will be occurring. In addition to the presentation, attendees will be asked questions in order to gain targeted feedback on certain areas of the Community Integration and Habilitation (CIH) waiver. These questions will be an open discussion and are meant to structure the course of the event and are available on the CIH Transform webpage, along with the town hall meeting dates and PowerPoint presentation.
While DDRS understands that not all family members, participants or providers may be able to attend the meeting, a survey will be posted on the CIH Transform’s website in the near future that will allow stakeholders to provide input outside of the September meetings. DDRS values your input, feedback and ideas for transformation and looks forward to hearing from you. You may also submit questions or comments to CIHW@fssa.in.gov. If you are attending one of the town hall meetings and will need interpreter servicers or other accommodations you may email CIHW@fssa.in.gov with your request. Please provide 48 hours advance notice to the meeting, to accommodate the request when at all possible. Comments are closed.
