2016 Mortality Data Review Report available
The Bureau of Quality Improvement Services (BQIS), within the Division of Disability and Rehabilitative Services, monitors services to individuals with intellectual and developmental disabilities administered by organizations and providers throughout the state of Indiana. Part of the function of BQIS is to review the deaths of all individuals receiving services through the Bureau of Developmental Disabilities Services.
The purpose of the mortality review is to identify trends, develop recommendations and bring about improvement in both provider-specific and system wide service delivery. The information and data collected during the mortality review process is reflected in the annual Mortality Data Review Report.
